CAC meeting – Epidural interventions for chronic pain management: Voting questions nophi
Dear CAC representatives and colleagues,
In preparation for our meeting tomorrow, we are providing you with the Survey Monkey form through which you can provide answers to Voting Question that will be discussed on the call tomorrow.
Only the selected panelists will be permitted to speak during the meeting. However, CAC members have the opportunity to respond to these voting questions using the following link: https://www.surveymonkey.com/r/MJCAC_epidural. The Survey Monkey form will be activated at the start of the meeting and responses can be submitted until end of day on Friday, February 19, 2021.
The questions will be discussed by the selected panelists during the meeting, but CAC members and other attendees will not be able to send comments or answers via WebEx chat during the meeting. Your responses will become part of the meeting/LCD record and will be accessible for review by the Contractor Medical Directors (CMDs).
The Survey Monkey voting question results will be posted to our website within 3-4 weeks after the meeting. We will send these results to all of you as well.
